Reflexiones en torno a los problemas éticos y bioéticos en el tratamiento del paciente en fase terminal Reflections about the ethical and bioethical problems in treatment presenting with a terminal disease

Abstract

Se realizó una revisión bibliográfica sobre aspectos éticos y bioéticos en el tratamiento del paciente en estado terminal. Es un derecho de todos los ciudadanos que se les atienda y proteja su salud, teniendo como referencia el marco legal y el concepto actual de salud, en el que cobran vigencia los 3 principios fundamentales que forman el triángulo bioético: autonomía, justicia y beneficencia. La actuación ética con estos pacientes que reciben cuidados paliativos no difiere de los principios rectores de cualquier otro acto relacionado con la atención médica,ya que toda acción debe estar dirigida al beneficio del ser humano sin provocar daños. Existe considerable confusión y controversia en cuanto a la ética del cuidado terminal, particularmente en las áreas más polémicas de resucitación, en el inicio y cese de tratamientos, la alimentación y la eutanasia. El enfermo terminal tiene derecho a vivir hasta su máximo potencial físico, emocional, espiritual, vocacional y social, compatible con el estado resultante de la progresión de la enfermedad. Estos son los aspectos que se tratan en este trabajo.<br>A bibliographic review was conducted about ethical and bioethical features in treatment of patient presenting a terminal disease. It is a right of all citizens the care and protection of his health within the legal framework and the current concept of health, with validity of the three fundamental principles included in bioethical triangle: autonomy, justice and charity. Ethical conduct with these patients receiving palliative cares not differs of the governing principles of any act related to medical care, since all action must to be directed to benefit of human being without cause damages. There is much confusion and controversy as regards the terminal care ethics, particularly in the more controversial areas of resuscitation, at onset and end of treatments, feeding and euthanasia. Patient presenting with a terminal disease has the right of live until its maximal physical emotional, spiritual, vocational and social potential compatible with the resulting state of disease progression. These are the features approached in present paper.
