Nurses' knowledge of heart failure in a North Carolina community hospital

Heart failure (HF) is an epidemic affecting approximately 5.1 million people in the United States and is one of the most expensive cardiovascular illnesses, costing the nation an estimated $32 billion each year (CDC, 2013). There is no cure for HF, but following guidelines established by the American Heart Association can contribute to decreased mortality, improved quality of life, and decreased hospital admissions. Nurses play an essential role in providing comprehensive HF self-management education to patients, and thus, should be knowledgeable about HF principles themselves. Research suggests that nurses may not have adequate knowledge of HF management to educate patients, and consequently, are not optimally prepared to provide HF education (Hart, Spiva, & Kimble, 2011).This was an exploratory, descriptive study with a survey design seeking to describe nurses’ knowledge of HF self-management principles within a community hospital in North Carolina. The Nurses’ Knowledge of Heart Failure Education Principle questionnaire developed by Dr. Nancy Albert (2002) was utilized to determine which HF education topics nurses were knowledgeable about and which topics they were deficient. Thirty-two registered nurses agreed to participate, with a mean knowledge score of 15.75 (78.75% correct). Consistent with previous studies, nurses scored lowest on knowledge related to transient dizziness (28.1% correct), asymptomatic hypotension (18.8% correct), daily weight assessment (25% correct), and salt substitutes (53.1% correct). Findings confirm previous work suggesting that nurses may not be adequately prepared to educate patients with HF about self-management.This aspect of nursing education is important, because nurses caring for HF patients need to have sufficient knowledge of HF self-management principles in order to provide optimal education to patients. Lack of adequate education for HF patients results in noncompliance with outpatient treatment modalities and ultimately leads to hospital readmissions related to HF exacerbation. Nurses with adequate knowledge of HF can provide better education to patients on how to manage their HF, potentially reducing hospital readmission rates related to HF and improving patients’ overall quality of health.Date

However, students' specific uses of analogies for constructing arguments are not well understood. Specifically, the results of preservice teachers' knowledge gains are not widely studied. Although we would hope that engaging preservice science teachers in exemplary lessons would assist them in using and generating analogies more expertly, it is not clear whether or how such curricula would affect their learning or teaching. This study presents an existence proof of how preservice science teachers used analogies embedded in their course materials Physics by Inquiry. This fine-grained analysis of small group discourse revealed three distinct roles of analogies including the development of: (a) cognitive process skills, (b) scientific conceptual understanding, and (c) social contexts for problem solving. Results suggest that preservice teachers tend to overgeneralize the analogies inserted by curriculum materials, map irrelevant features of analogies into collaborative problem solving, and generate personal analogies, which counter scientific concept development. Although the authors agree with the importance of collaborative problem solving and the insertion of analogies for preservice teachers' conceptual development, we believe much more needs to be understood before teachers can be expected to construct and sustain effective learning environments that rely on using analogies expertly. Implications for teacher preparation are also discussed.

Examining the anatomy of a screencast: Uncovering common elements and instructional strategiesSugar, William; East Carolina University; Brown, Abbie; East Carolina University; Luterbach, Kenneth; East Carolina University (AU Press, 2010-08-19)The researchers engaged in cooperative inquiry in order to explore screencasts as online instructional tools. In total, each researcher analyzed 37 screencasts, which provided over two hours of instruction. The content area of these screencasts concentrated on teaching specific computing procedures (e.g., how to install web server software or how to add a table in a word processor). The researchers analyzed their own self-produced screencasts as well as those that were professionally produced. Analyses of the screencasts led the researchers to discover common structural components (i.e., bumpers, screen movement, and narration) and common instructional strategies (i.e., provide overview, describe procedure, present concept, focus attention, and elaborate content). By synthesizing the common structure and common instructional strategies, the researchers offer a framework for considering the role of screencasts as online instructional tools. To introduce a practical application of the framework, the researchers created a screencasting checklist, which may be used by online instructors and instructional designers to develop and assess their own screencasts. This initial work invites additional research and development in order to refine the screencasting framework and checklist.
