Abstract
The purpose of this systematic review is to inform the World Bank, development partners and other stakeholders on the state of governance in the health sector of Bangladesh. With billions2 of Bangladeshi Taka invested in the government-run health sector every year, financed by domestic and international sources, there have been numerous efforts to better understand the governance challenges in the sector. This report reviews sixty such reports and studies written by NGOs, academics, government entities, research institutions, consultants and development partners, in the last fourteen years. However, translating that research into policy dialogue and improvement has been challenged due to poor knowledge management, a lack of a systematic approach to research (building on earlier studies findings), and limited efforts to place the work in the public domain and discourse. This paper takes a systematic approach to the literature review, bringing the key findings together and evaluating the weight of evidence on the governance problems in the sector, as well as the gaps that should be addressed by future research efforts. This review furthermore emphasizes the need to understand the factors, and political economy factors in particular, that explain the current state of affairs. The review is structured to systematically assess this literature, leading to several broad conclusions. First, it identifies the structure and method of the review and outlines the categorization of the governance issues reviewed, and provides a background to the health sector. It then methodically summarizes the relevant literature for each governance issue, highlighting the lessons and identifying the gaps and weaknesses. The review ends by highlighting these gaps in research, with a view to improve its scope and impact.

    Improving the Quality of Public Expenditure in the Dominican Republic

    This book addresses the achievements,
 challenges, and opportunities to improve the quality of
 public spending. Steps to make such changes have come
 through monitoring and evaluation approaches that can be
 replicated or expanded; sectoral efforts to improve the
 performance of priority programs; Congress's use of
 information on the results of public spending; the
 implementation of performance budgeting at subnational
 levels; and the harmonization of accounting between the
 three levels of the federal government. All these aspects
 are key elements of comprehensive reform. Currently, as the
 book states, accountability focuses on achieving results
 rather than on centering attention on mere compliance with
 rules and procedures. In this context, based on a new legal
 framework, the government of Mexico has decisively promoted
 results-based management and budgeting. The Performance
 Evaluation System (SED) was finally established in 2008 with
 the institution of the principles, concepts, methodologies,
 guidelines, procedures, and systems that support its
 operation. Its adoption as a common practice in the Federal
 Public Administration (APF) process will require a gradual,
 progressive, systematic learning and continuous improvement
 that should allow performance evaluation to take root in the
 APF. This calls for consolidating the Results-Based
 Budgeting (RBB)-SED in all agencies, expanding its use and
 improving the quality of the information that feeds it.
 However, not just the APF benefit will from the
 implementation of the RBB-SED. As the publication suggests,
 the approach to an expenditure budget based on performance
 information offers Congress great opportunities to enhance
 its regulatory and supervisory functions. The improvement in
 the quality of Matrices de Indicadores para Resultados
 (MIRs), program evaluations, and their integration into the
 budgetary programming cycle also contributes to this purpose.

    Strengthening Local Government Budgeting and Accountability

    In many developing and middle-income countries, decentralization reforms are promoting changes in governance structures that are reshaping the relationship between local governments and citizens. The success of these decentralization reforms depends on the existence of sound public financial systems both at the central and local levels. This paper focuses on the role of budgeting as a critical tool in reform efforts, highlighting problems that might impede successful local government budget development and implementation. The attainment of effective local government accountability and transparency is not an end itself, but rather it represents the means to support better decision-making on national and local budgeting. Community based schemes for enhancing local government accountability need to combine legal, political, and administrative mechanisms with proactive community involvement. Of particular importance are the legal and budgetary instruments that require input from local community members on certain local government decisions and instruments that increase accessibility for the press or the general public at large to information on government activities.

    Governance in Health Care Delivery : Raising Performance

    The impacts of health care investments
 in developing and transition countries are typically
 measured by inputs and general health outcomes. Missing from
 the health agenda are measures of performance that reflect
 whether health systems are meeting their objectives; public
 resources are being used appropriately; and the priorities
 of governments are being implemented. This paper suggests
 that good governance is central to raising performance in
 health care delivery. Crucial to high performance are
 standards, information, incentives and accountability. This
 paper provides a definition of good governance in health and
 a framework for thinking about governance issues as a way of
 improving performance in the health sector. Performance
 indicators that offer the potential for tracking relative
 health performance are proposed, and provide the context for
 the discussion of good governance in health service delivery
 in the areas of budget and resource management, individual
 provider performance, health facility performance, informal
 payments, and corruption perceptions. What we do and do not
 know about effective solutions to advance good governance
 and performance in health is presented for each area,
 drawing on existing research and documented experiences.
